INTAKE FORM
A completed intake form is essential in order identify and limitations and/or contraindications. Your health is our priority and we strive to offer services that align with the information you provide. For those reasons, intake forms are required to be submitted prior to your scheduled appointment. Clients under the age of 18 must have written parental or guardianship consent for all services, no exceptions.
CONTRAINDICATIONS
Please notify one of our specialists before reserving your treatments if you have high blood pressure, physical limitations, diabetes, are pregnant, or require special accommodations. Extended exposure to hot water vapors may be detrimental to the health of those with diabetes, high or low blood pressure, and heart conditions. If you feel you have a medical condition or are currently under medical supervision, it is recommended that you consult your physician before receiving services.

NEED TO CANCEL?
We know that life sometimes gets in the way. For that reason, we try to be as flexible as possible when it comes to cancelling appointments. If you cancel more than 24 hours before your set appointment, we’ll do our best to set up a better slot for you, no questions asked. If you cancel with less than 24 hours before your appointment, we will charge an administration fee.
In the case of repeated no-shows or last-minute cancellations, you will be required to prepay your service in full to guarantee future reservations.

PRIVACY
Any and all client information is private, confidential, and will be treated in accordance with HIPPA regulations.
ACCESSABILITY
Our service room is handicap-accessible. Service techs are not allowed to lift people onto tables, beds, or chairs. If assistance is required, we kindly ask for you to bring a personal care attendant. Please let us know when making your appointment if you require accessibility.
CONTRAINDICATIONS

Please note that late arrival may reduce the allotted time scheduled for your appointment, so as not to delay the next scheduled guest. Although its not usually an issue, parking in our area is scarce. Arriving 15 minutes early for your appointment gives you time to find parking, relax and unwind. We want you to enjoy your full experience. Arrival past 20 minutes of scheduled appointment will be considered a missed appointment and, if applicable, forfeiture of deposit. Now, we love accountability here, so if your specialist is 15 minutes late, you will receive a 15% discount on your service in appreciation for your patience.
REFUSAL OF SERVICE
Although we hope to never have to implement this policy, we find it important and necessary for unforeseen circumstances. We reserve the right to refuse or discontinue service to anyone demonstrating behavior that is perceived to be inappropriate or disruptive to our atmosphere.
